Anxiety Answer:

There is one called the Sports Competition Anxiety Test (SCAT) by Rainer Martens. This is short, easily scored, and straightforward. You can find this in a book by Martens, 1977, with the same title.

Other tests include the Competitive State Anxiety Inventory (Martens, Vealey, & Burton, 1990), and the Sport Anxiety Scale (Smith, Smoll, & Schutz, 1990).

Most texts on sport psychology should have references for them, and frequently examples of the items.
